What Documents Must Be Included in a Driver Qualification File?
A Driver Qualification File, or DQ file, contains documents demonstrating that a commercial driver is qualified to operate a commercial motor vehicle.

Core Driver Qualification Documents
Driver’s Application for Employment
The application should contain the information required by 49 CFR §391.21, including the driver’s employment and driving history.
Initial Motor Vehicle Record
The carrier must obtain the driver’s motor vehicle record from each applicable state licensing authority as required.
Previous Employer Safety Performance History
The carrier must investigate the driver’s safety performance history with applicable DOT-regulated employers from the preceding three years.
Road Test Certificate or Equivalent
The driver must complete a road test unless the carrier properly accepts an allowable equivalent, such as an applicable CDL.
Medical Examiner’s Certificate
When applicable, the carrier must verify that the driver is physically qualified to operate a commercial motor vehicle.
Medical Examiner Verification
When required, the carrier should verify that the medical examiner was listed on the National Registry of Certified Medical Examiners on the date of the examination.
Annual Motor Vehicle Record Review
The carrier must obtain and review the driver’s motor vehicle record at least annually and document the review.
Annual Review of Driving Record
The company must document that it reviewed the driver’s record and determined whether the driver continues to meet qualification requirements.
Required Waivers or Exemptions
Any FMCSA waiver, exemption, or Skill Performance Evaluation certificate applicable to the driver should be maintained in the file.

Related Records Usually Maintained Separately
Some documents relate to the driver but should generally be stored separately from the standard DQ file because of confidentiality or separate recordkeeping requirements. These may include:
Drug and alcohol testing records.
Clearinghouse query records.
Federal custody and control forms.
Alcohol testing forms.
Employment tax and payroll documents.
Direct deposit information.
Why Complete DQ Files Matter
Missing or incomplete driver qualification records are common findings during FMCSA Safety Audits. A carrier should review every driver file before allowing the driver to operate and periodically audit the file for expired or missing documents.
